Follow Rock Quarry Road until it turns into Tarboro Road. Horizon Health Center is located at the corner of Boyer and Tarboro Roads in a gray one level building on your right.
Services Provided
Comprehensive primary care services are provided by a Family Practice Physician, a Family Nurse Practicioner,a Registered Nurse and a Licensed Clinical Addiction Specialist. Services include acute care, management of chronic health conditions including HIV, referrals to medical specialists, post-hospital and surgical follow up, access to medications, labs, diabetes education and substance abuse services.
General Primary Medical Care including: Complete Physicals, Diagnostic Tests/Screens, Family Planning, Geriatric Care, Gynecological Care, Hearing & Vision Screening, Immunizations, Pediatric Care.
Outreach Centers
Horizon's staff also operates two outreach sites:.
Oberlin Outreach Center at Interact's Family Safety and Empowerment Center provides medical services for victims of domestic violence.
South Wilmington Outreach Center provides medical care to homeless men at the South Wilmington Street men's shelter.
Same-Day Care (temporary location within Rock Quarry Road Family Medicine, Raleigh)
When minor illness strikes suddenly and you need an appointment today, we offer same-day appointments for existing Wake Health Services patients, age 12 and up. Located within Rock Quarry Road Family Medicine, 1001 Rock Quarry Road, Raleigh (temporary location), Same-Day Care hours are 12 noon to 8 pm Monday through Friday. Same-Day appointments are for minor injuries and illnesses only. Please continue to see your primary care provider at Wake Health Services for chronic illnesses such as diabetes, high blood pressure and other on-going conditions. Support Services
Horizon Health Center founded and actively participates in a county-wide diabetes collaborative addressing education, access to medications, diabetes supplies and specialty shoes. Horizon provides HIV adherence counseling through a partnership with Alliance of AIDS Services Carolina. Horizon collaborates with area shelters to identify medically at risk individuals who need shelter during extreme weather as well as medical respite coordination. Horizon works with pharmaceutical company programs to meet patients' medication needs. Horizon coordinates care with wrap-around mental health agencies and is an active participant with the local coalition to end homelessness.

Horizon’s Homeless Art Project
Horizon's Homeless Art Project began when practice manager David Engel gave disposable cameras and journals to five homeless patients and asked them to document life on the streets through their eyes. What resulted was a journey of images, observations and inspirations that far exceeded anyone’s expectations. The patients commented that participating in this project boosted their self-worth and was “more helpful than years of therapy.” Yes, there is depression, drug abuse and hopelessness, but there is also a sense of inner beauty, insight and dignity, despite— and maybe because of— their circumstances. Some artists wanted to remain anonymous; others willingly signed their pictures and their prose; all wanted to share their journeys in hopes of connecting and moving between their world and ours. Warmest thanks to John Catalano who created the composites, and most of all, deepest appreciation to the artists who shared their lives. Horizon Health Center is a healthcare for the homeless program and a division of Wake Health Services, Inc., a Community Health Center in Raleigh, N.C

James Hartye, MD
Dr. Hartye joined Wake Health in July, 1990. He received his undergraduate degree from Villanova University (Villanova, PA) in 1973 and his Doctor of Medicine degree from Vanderbilt University Medical School (Nashville, TN) in 1977. Dr. Hartye completed a two-year residency in Family Medicine at Duke University Medical Center (Durham, NC) in 1979 and a one-year residency in Family Medicine at Central Texas Medical Foundation (Austin, TX) in 1980. He served as Medical Director from 1990 – 1995, and again as Interim Medical Director in 2003. Dr. Hartye is board certified in Family Medicine.
Kate Henderson, MSN, FNP
Kathryn M. Henderson, MPH, MSN, FNP Kathryn joined Wake Health in July, 2009. She received her undergraduate degree in Environmental Health from Colorado State University (Ft. Collins, CO) in 2000. Kathryn received her Master of Public Health degree in Epidemiology from the George Washington University (Washington, DC) in 2006. She also received her Master of Science degree in Nursing from Virginia Commonwealth University (Richmond, VA) in 2008. Kathryn is board certified as a Family Nurse Practitioner.
Catherine Thrash, LCSW, LCAS
Catherine A. Thrash, LCSW, LCAS Catherine joined Wake Health in August, 2004. She received her Masters degree in Accounting from Mississippi State University (Starkville, MS) in 1984.
