We are now accepting NEW PATIENTS. Make an appointment by calling 919-833-3111.  We accept all major insurances, but if you are uninsured we offer a Discount Fee Program. Please download an application and follow the directions for  Making An Appointment.

View Map. Take I-40 towards Raleigh to Rock Quarry Road (exit 300). Make a left at the end of the exit ramp. At the second stop light, veer to the left on to Rock Quarry Road. Rock Quarry Road Family Medicine is the first building on the right.

Sherri’ James-Surgers, MD, Chief Medical Officer

Dr. Sherri’ James-Surgers comes to Wake Health as Chief Medical Officer, and brings more than 15 years of diverse leadership and practical experience as both a medical director and a practicing clinician. She managed physicians and clinicians in several medical sectors including community/public health, rural medicine, occupational health, solo-private and large group practice. Married with three children, Dr. James-Surgers recently completed a Masters Degree in Healthcare Management. "In addition to my kids, the accomplishments I'm most proud of are improving patient outcomes and patient satisfaction, and coaching medical staff." 

Sharon (Sam) Cullinan, MSN, FNP

Sam joined WHSI in October 1996. She received her Master of Science degree in Nursing from the University of North Carolina (Chapel Hill, NC) in 1996. She received a Bachelor of Science degree in Nursing from the University of North Carolina in 1990 and a diploma in Nursing from the Maryview Hospital School of Nursing (Portsmouth, VA) in 1968. Sam is board certified as a Family Nurse Practitioner.

Anthony Robinson, PA

 Anthony received a Bachelor of Science degree in Biology from the University of North Carolina (Chapel Hill, NC) in 2004. Anthony received his Master of Health Sciences degree and Physician Assistant Certificate from the Duke University Physician Assistant Program (Durham, NC) in 2007.

Sara Johnston, PA

Sara has practiced in tthe areas  of  Internal Medicine and Cardiology/Cardiothoracic Surgery.  She is a graduate of  Wake Forest University with a Master of Medical Science Degree in Physician Assistant Studies. 

Patricia Rickenbaker, LCSW

Patricia Rickenbaker is a Licensed Clinical Social Worker with 16 years of social work experience and has spent several years working in a rural health clinic in South Carolina. She earned a graduate degree in clinical social work from the University of North Carolina at Chapel Hill and trained at Duke Psychiatric Outpatient Clinic. She completed three years post-graduate training at the Southeast Institute for Groups and Families. She trained for seven years in methods of trauma recovery with the Foundation for Human Enrichment and is now a trainer in those methods. She is a member of the American Academy of Psychotherapists and the National Association of Social Workers.

Doris Nicolas-Mir, Registered Dietician

 Prior to joining Wake Health, Doris worked at the Aramark-Johnston Medical Center as their Clinical Dietician. She received her undergraduate degree from Florida International University, and her Masters of Public Health from the University of Miami.   She is fluent in Spanish, as well as English. 

Ismael Tamba, DO

Dr. Ismael Tamba will be joining Wake Health in October, 2010.  Dr. Tamba is board-certified in Osteopathic Medicine. He received his undergraduate degree from the University of Rochester, NY, his Masters degree of Science in Public Health from Tulane University, and his Doctor of Osteopathic Medicine from the University of Medicine and Dentistry of the New Jersey School of Osteopathic Medicine. Dr. Tamba completed an internship at the Crozer Keystone Health System in Pennsylvania and his residency program in Family Medicine & Community Health at the University of Pennsylvania Health System.  Dr. Tamba is fluent in French, as well as English, and speaks conversational Spanish.
