Wake Health Services
has relationships with many area health care organizations so that
patients get the services they need.
These relationships include many important aspects of patient care:
WHSI collaborated with
the State of North Carolina, NC Medical Society, WakeMed and Wake
County Human Services to form a Carolina Access II case management
program for Medicaid patients.
WHSI collaborates with Wake
Smiles, a volunteer program of the Raleigh/Wake County Dental Society,
to provide space in the evenings to local dentists who volunteer services
to indigent patients.
Project DIRECT, a
collaborative diabetes research and education program, provides on-going
diabetes education classes at Rock Quarry Road Family Medicine, an
office of Wake Health Services.
WHSI collaborates
with Wake County Human Services to have county staff at our sites
who directly enroll patients in Medicaid and NC Health Choice.
WHSI refers patients
to the hospitalists at WakeMed Raleigh and Wake Med Cary.
Needed medicines are made
accessible to homeless patients through a federal drug-pricing contract
with Person Street Pharmacy.
WHSI has referral relationships
to and from Wake County Department of Mental Health.
WHSI has referral relationships
for x-rays at WakeMed and WakeMed Cary Hospital and for mammograms
at Wake County Human Services and Rex Healthcare.
WHSI is a main access
point for indigent patients enrolled in Project Access, a project
of the Wake County Medical Society, which provides a system of health
care for uninsured, low-income residents of Wake County through volunteer
physician services.
